Job Summary

We are seeking a skilled RN Navigator to join our team. The RN Navigator will serve as the primary point of contact for coordinating care and services for identified patient populations across the continuum of care. This role requires strong psychosocial assessment and clinical skills, with a focus on oncology and/or case management, care coordination experience.

Responsibilities
  • Coordinate care and services for patient populations across the continuum of care
  • Assume a leadership role with the multidisciplinary team to identify and eliminate barriers to care
  • Promote effective resource utilization and achieve optimal clinical outcomes for patients and caregivers
Requirements
  • Three years of experience in nursing, with one year in a specialty
  • Bachelor's Degree in Nursing required
  • Registered Nurse License required
  • Basic Life Support through the American Heart Association required
  • Nursing Professional Certification through the American Nurse Credentialing Center preferred
