Care Navigator Per Diem

The Care Navigator is a key member of our Integrated Care team, working closely with Primary Care Providers, Behavioral Health Clinicians, and psychiatric providers to coordinate and support mental health care within our clinic and beyond. This role requires a strong understanding of human services and medical-related fields, with a minimum of 2 years of experience in client/patient interfacing.
Key Responsibilities:
- Support and coordinate behavioral health care with patients, Primary Care Providers, BHCs, psychiatric providers, and other Integrated Care team members.
- Provide patient education on common mental health and substance abuse disorders, as well as available treatment options.
- Monitor patients for changes in clinical symptoms and treatment side effects or complications.
- Facilitate referrals to mental health clinics and substance use disorder treatment programs as clinically indicated.
- Participate in regular Integrated Care team meetings and communicate treatment recommendations to team members and outside collaborators as needed.
- Handle confidential information with sensitivity and discretion.
Requirements:
- AS degree (required)
- Minimum 2 years of experience in human services or medical-related field, with client/patient interfacing experience
Physical Requirements:
Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.
