Job Summary

The Direct Support Professional is a vital member of our team, responsible for providing direct oversight, guidance, and training to individuals receiving supports and services. This role requires a unique blend of compassion, patience, and professionalism, as you work to empower individuals to reach their maximum potential and promote independence and community integration.

Key Responsibilities
  • Provide personalized supports, assessments, and training to individuals, tailored to their unique needs and goals.
  • Ensure a safe and supportive environment, adhering to individual Safeguards Plans, Individual Plans of Protection, and other relevant plans.
  • Accurately document services and maintain accurate records, in accordance with agency, OPWDD, Medicaid, and other funder requirements.
  • Protect health management information, consistent with HIPAA guidelines.
  • Participate in the development and implementation of Staff Action Plans, in collaboration with individuals, families, advocates, and Care Coordinators.
  • Maintain knowledge of individual medical, physical, psychological, and dental health care needs.
  • Recognize and respond to signs and symptoms of illness or injury, and notify supervisors or nurses as necessary.
  • Provide personal hygiene and health care assistance, as needed, including toileting, showering, oral hygiene, and other essential tasks.
  • Administer medications, as trained and designated, in accordance with agency policy and procedure.
  • Support individuals in receiving needed medical services, communicating and documenting pertinent information as required.
  • Ensure the safety of all individuals in everyday situations, working collaboratively with the team to promote positive behavioral strategies.
  • Provide transportation, as necessary, following proper safety procedures.
  • Assist individuals in building positive relationships within their community, supporting their personal choices and advocacy efforts.
  • Communicate professionally with colleagues, individuals, families, and the community, as necessary.
Requirements
  • High school diploma or equivalent.
  • Valid NYS driver's license with a safe driving record, per agency policy.
Working Conditions

This role may involve frequent travel to off-site locations, including job/volunteer sites, community recreational areas, and personal residences. Evening and weekend work may be required, and the use of personal vehicles may be necessary.
