Cardiovascular Patient Navigator

We are seeking a skilled Cardiovascular Patient Navigator to join our team at Duke Health. In this role, you will be responsible for providing direct patient care services to patients with cardiovascular diseases.
As a Cardiovascular Patient Navigator, you will work closely with our multidisciplinary team to develop and implement individualized treatment plans for patients with cardiovascular diseases. You will also be involved in the training of new employees, students, and fellow trainees.

Responsibilities:
- Evaluate new patients and consult with established patients on the cardiovascular service.
- Develop and implement treatment plans for patients with cardiovascular diseases.
- Work collaboratively with the inpatient rounding teams to allocate workload based on daily census and clinical needs.
Requirements:
- Graduate of ARC-accredited Master's Degree or Doctoral Degree Physician Assistant program.
- Unrestricted medical license issued by NCMB to practice as a PA.
- Federal and State DEA with full prescriptive authority.
Salary: The estimated salary for this position is $130,000 - $160,000 per year, depending on experience.
