About the Role:
Kaiser Permanente is seeking a skilled Social Work Licensed Clinical Social Worker to join our team in a full-time capacity. As a key member of our healthcare team, you will provide essential social work services to patients and their families, ensuring comprehensive care and support.
Key Responsibilities:
- Provide social work services, including assessment, care planning, and case coordination to patients and their families.
- Offer consultation to staff and other healthcare providers to ensure seamless care delivery.
- Develop and implement social work plans of care, ensuring patient-centered and evidence-based practices.
- Serve as a liaison between Kaiser Permanente and the community, integrating services, coordinating care, and enhancing relationships through public relations activities.
- Prepare and evaluate assessments, develop and implement social work service plans, and provide psychosocial assessments and diagnostic impressions.
- Help patients and families enhance or restore personal and social functioning, and perform case management, coordination, and casework intervention.
- Assist with the administration, development, implementation, and evaluation of social work programs and policies, and conduct social work consultation and resource development.
- Participate in performance improvement, implement Quality Improvement processes, and assist with the development and implementation of departmental annual work-plans.
- Conduct research through formal design/methodology of data collection and analysis/evaluation of data, social work programs, social systems, and social policies.
- Document according to departmental standards, Federal and State regulations, and assure medical record documentation.
Requirements:
- Masters degree in social work.
- License, Certification, Registration: Healthcare experience working with medical diagnoses, demonstrated ability in networking with community agencies, and minimum one (1) year of social work experience.
