Position Overview:
The Pharmaceutical Care Specialist is a licensed expert responsible for delivering safe and effective pharmacy services to patients in alignment with the policies and procedures of Option Care Health Inc., professional pharmacy standards, and relevant regulatory bodies.
The Pharmaceutical Care Specialist collaborates with the Nursing Department to coordinate patient services, track patient progress towards health objectives, and educate patients, caregivers, staff, and other healthcare professionals on appropriate medication management.
Key Responsibilities:
Patient Assessment:
Evaluates the appropriateness of individual patients for home care services. Gathers and organizes all necessary patient-specific information to assess their suitability for home care. Reviews medical conditions and prescribed medication therapies to ensure they align with home care service requirements and expected outcomes.
Pharmacy Care Planning:
Develops, recommends, monitors, and assesses patient-specific pharmacy care plans tailored to the pharmacotherapy needs of home care patients.
When necessary, suggests modifications to prescribed medication therapies or monitoring parameters for home care patients.
Collects and organizes patient-specific data to prevent, identify, and resolve medication-related issues and to provide suitable medication therapy recommendations.
Identifies appropriate monitoring parameters for each patient and conducts nutritional assessments, making relevant recommendations.Adjusts pharmacy care plans for home care patients based on evaluations of monitoring data and other pertinent patient-specific information.
Demonstrates proficiency in pharmacokinetic principles and the interpretation of laboratory values.Education and Training:
Provides medication-use education to Option Care staff, other healthcare professionals, home care patients, and their caregivers. Employs effective patient education strategies to counsel home care patients and their caregivers.
Continuity of Care:
Ensures seamless pharmaceutical care transitions between home and other patient-care settings. Utilizes a systematic approach to communicate relevant patient information across care settings. Prepares and dispenses medications adhering to established techniques and Option Care policies. Ensures equipment is accurately calibrated and medications are prepared with the correct concentration, stability, and storage conditions. Complies with safety and quality assurance protocols, ensuring labels meet organizational standards.
Adheres to state and federal regulations, including those set forth by the Drug Enforcement Administration (DEA) regarding controlled substances.
Maintains compliance with policies governing the perpetual inventory systems for controlled substances and other medications.
Equipment Management:
Oversees the use, maintenance, and troubleshooting of medication administration and related equipment utilized in managing home care patients.
Accurately programs, maintains, and advises others on the operation of all medication administration and related equipment used by Option Care.
Participates in the assessment, management, and reporting of medication-related incidents and equipment errors. Completes quality assurance documentation in accordance with Option Care policy. Monitors and reports discrepancies in perpetual inventory systems for controlled substances.Drug Information:
Provides timely and relevant responses to drug information inquiries from healthcare providers and home care patients. Formulates responses based on a thorough analysis of current literature.
Engages in educational seminars or continuing education opportunities to enhance professional competency and meet state-mandated continuing education requirements.
Manages investigational drug products in accordance with established regulations and Option Care policies. Effectively utilizes pharmacy support personnel, prioritizing tasks and organizing workflow to maximize available resources. Ensures the accuracy of work performed by pharmacy support staff.Compliance and Confidentiality:
In all activities, adheres to accreditation, legal, regulatory, and safety standards. Maintains the confidentiality of patient and proprietary information, following legal and ethical guidelines for safeguarding sensitive data.
Supervisory Responsibilities:
This position does not include supervisory responsibilities.
Qualifications:
Graduate of an accredited School of Pharmacy.
Current, active Pharmacy license issued by the State Board of Pharmacy.
Experience:
One of the following:
(a) At least one year of experience in sterile IV admixture procedures (b) knowledge of non-sterile compounding, immunizations, and medication therapy management (MTM) (c) completion of an ASHP accredited Pharmacy Residency Program, (d) completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy or (e) equivalent experience as approved by the Vice President (VP) of Pharmacy and/or Sr. Vice President (SVP) of Clinical Services.
Basic Qualifications:
Experience in delivering customer service to both internal and external customers, including meeting quality service standards and evaluating customer satisfaction.
Basic proficiency in Microsoft Word, Excel, and PowerPoint.
Travel Requirements:
Willingness to travel up to 5% of the time for business purposes (within state).
Preferred Qualifications:
Completion of an ASHP accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy. Experience in home care, hospital, or other clean room settings.
Management Experience:
At least one year of experience in indirectly managing team members, including assisting in the development, training, and assignment of work/projects to other team members.
Salary:
Salary will be determined based on the applicant's education, experience, knowledge, skills, and abilities, as well as internal equity and market alignment. Minimum pay is $95,208.92+.
