Key Responsibilities:
- Collaborate with physicians, staff, and other healthcare professionals to provide comprehensive care coordination.
- Work closely with the Practice Transformation Quality team to ensure high-quality standards of care are maintained.
- Coordinate self-management support and disease registry activities for the identified population.
- Utilize data to inform health management decisions and improve patient outcomes.
Requirements:
- Current Washington State RN license.
- Minimum three years' experience in a hospital, home care, or managed care setting.
- Bilingual and/or English-Spanish speaking abilities preferred.
