Job Summary:

The Case Management Nurse provides comprehensive care coordination and case management for a diverse panel of patients, focusing on improving health behaviors and outcomes. This role involves working as part of an interdisciplinary healthcare team to guide patients in reaching their health-related goals.

Key Responsibilities:

Deliver comprehensive case management for patients with varying health conditions and demographics.

Act as a "coach" to patients, encouraging health behavior changes to achieve health goals.

Collaborate with providers, social workers, patients, caregivers, and other health professionals.

Travel to hospitals, skilled nursing facilities, and patient homes as required.

Required Qualifications:

Education: Graduate from an accredited school of nursing.

Licensure: Active, unrestricted RN license in the state.

Additional Requirements: COVID-19 vaccination, valid driver's license, and ability to travel.

Preferred Qualifications:

Certification in Geriatric Nursing or Geriatric Case Management.

5+ years of nursing experience, preferably with older adults.

Proficiency in computer use, health information technology, and strong interpersonal skills.
