About the Role

Job Summary

This office-based position involves coordinating patient care, ensuring quality assurance standards, and providing administrative support. The ideal candidate will have experience in hospice care, strong organizational skills, and a commitment to delivering exceptional patient care.

Key Responsibilities:
  • Ensure compliance with regulatory and accreditation standards.
  • Conduct regular audits of patient charts and documentation.
  • Identify areas for improvement and implement corrective actions.
  • Participate in QA meetings and contribute to policy development.
Administrative Support:
  • Maintain accurate and up-to-date patient records.
  • Assist with onboarding and training new staff.
  • Handle patient inquiries and provide information about services.
  • Collaborate with the billing department for accurate and timely billing.
Communication:
  • Serve as a liaison between patients, families, and healthcare providers.
  • Provide education and support regarding care plans and treatment options.
  • Communicate effectively with the healthcare team for coordinated care.
Requirements

To be successful in this role, you will need:

  • A current and valid LVN license in the state of employment.
  • Minimum 2 years of experience in hospice and home health preferred.
  • Experience with quality assurance processes and regulatory compliance.
What We Offer
