Hospice Case Manager Professional

We are seeking a highly skilled and compassionate Hospice Case Manager to join our team at A Hug Away Healthcare, Inc. This is an excellent opportunity for a dedicated professional to make a meaningful difference in the lives of our patients and their families.
Job SummaryThis office-based position involves coordinating patient care, ensuring quality assurance standards, and providing administrative support. The ideal candidate will have experience in hospice care, strong organizational skills, and a commitment to delivering exceptional patient care.
Key Responsibilities:- Ensure compliance with regulatory and accreditation standards.
- Conduct regular audits of patient charts and documentation.
- Identify areas for improvement and implement corrective actions.
- Participate in QA meetings and contribute to policy development.
- Maintain accurate and up-to-date patient records.
- Assist with onboarding and training new staff.
- Handle patient inquiries and provide information about services.
- Collaborate with the billing department for accurate and timely billing.
- Serve as a liaison between patients, families, and healthcare providers.
- Provide education and support regarding care plans and treatment options.
- Communicate effectively with the healthcare team for coordinated care.
To be successful in this role, you will need:
- A current and valid LVN license in the state of employment.
- Minimum 2 years of experience in hospice and home health preferred.
- Experience with quality assurance processes and regulatory compliance.
