Job Description

At Memorial Hermann Health System, we are dedicated to delivering high-quality, patient-centered care. As a Clinical Care Coordinator, you will play a vital role in facilitating seamless transitions between hospital and post-acute settings, ensuring timely and efficient care for our patients.


Responsibilities:
  • Coordinate patient care progression throughout the continuum, working collaboratively with physicians, nurses, and other members of the healthcare team.
  • Develop and implement individualized continuing care plans in collaboration with patients, families, and healthcare providers.
  • Facilitate communication and coordination between healthcare providers, payors, and community resources to ensure effective care transitions.
  • Identify and address system problems impeding diagnostic or treatment progress, and develop solutions to resolve delays and obstacles to discharge.
  • Monitor length of stay and ancillary resource use, taking actions to achieve continuous improvement in both areas.
  • Collaborate with the clinical quality review department to identify potential issues and develop strategies to improve patient outcomes.
  • Ensure safe care to patients, staff, and visitors, adhering to all Memorial Hermann policies, procedures, and standards.
  • Participate in clinical performance improvement activities, contributing to the development and implementation of performance improvement strategies related to case management.

Requirements:
  • Graduate of an accredited school of professional nursing or graduate of an accredited Masters of Social Work program.
  • Current and valid license to practice as a Registered Nurse in the state of Texas or current and valid license as a Master Social Worker (LMSW) in the state of Texas.
  • Certification in Case Management required within two years of hire into the Case Manager position.
  • Three years of experience in hospital-based nursing or social work, with experience in utilization management, case management, discharge planning, or other cost/quality management programs preferred.
  • Excellent interpersonal communication and negotiation skills, with demonstrated leadership skills and strong analytical, data management, and PC skills.
