About the Role:

  • Maintain accurate and timely MDS assessments for all residents.
  • Collaborate with interdisciplinary teams to develop comprehensive care plans that meet individual resident needs.
  • Ensure seamless communication among staff members to ensure effective implementation of care plans.
  • Coordinate with ancillary departments to facilitate efficient resident care.

Responsibilities:

  1. Manage MDS assessments, ensuring accuracy and compliance with regulatory requirements.
  2. Develop and implement comprehensive care plans in collaboration with healthcare professionals.
  3. Communicate effectively with staff members to ensure consistent delivery of care.
  4. Facilitate regular RCP meetings to review resident care plans and make necessary adjustments.

Requirements:

  • RN license from the North Carolina Board of Nursing.
  • Experience with MDS-RAP and Care Planning functions.
  • Ability to read, understand, and adhere to policies and procedures.
  • Prefer experience with RUG-IV and MDS 3.0.

We offer a competitive salary range of $65,000 - $85,000 per year, depending on experience, along with benefits such as health insurance, paid time off, and opportunities for professional growth and development.
