About the Role:
- Maintain accurate and timely MDS assessments for all residents.
- Collaborate with interdisciplinary teams to develop comprehensive care plans that meet individual resident needs.
- Ensure seamless communication among staff members to ensure effective implementation of care plans.
- Coordinate with ancillary departments to facilitate efficient resident care.
Responsibilities:
- Manage MDS assessments, ensuring accuracy and compliance with regulatory requirements.
- Develop and implement comprehensive care plans in collaboration with healthcare professionals.
- Communicate effectively with staff members to ensure consistent delivery of care.
- Facilitate regular RCP meetings to review resident care plans and make necessary adjustments.
Requirements:
- RN license from the North Carolina Board of Nursing.
- Experience with MDS-RAP and Care Planning functions.
- Ability to read, understand, and adhere to policies and procedures.
- Prefer experience with RUG-IV and MDS 3.0.
We offer a competitive salary range of $65,000 - $85,000 per year, depending on experience, along with benefits such as health insurance, paid time off, and opportunities for professional growth and development.
