Position Overview:

The Case Manager plays a crucial role in evaluating, designing, and delivering integrated community-based services tailored to enhance independent living and improve individual, family, and community functioning for those with specific needs.

Key Responsibilities:

  2. Encourage positive interactions, demonstrating professional maturity, respect, and a commitment to a diverse workplace.
  3. Communicate effectively using agency communication systems.
  4. Manage information resources responsibly to meet the objectives of The Methodist Home.
  5. Support all activities necessary to achieve and maintain accreditation with regulatory bodies.
  6. Engage in professional development programs to enhance competency.

Supervisory Responsibilities:

  1. Foster innovation and goal-setting among clients.
  2. Encourage teamwork and collaboration.
  3. Ensure the overall well-being and support of clients, guaranteeing they receive adequate care.
  4. Provide training for new employees in collaboration with the Youth Services Coordinator and other staff.
  5. Assist in regular supervision meetings and performance evaluations of Youth Care Workers.

Specific Duties:

  1. Exhibit professionalism and leadership while understanding the program's objectives.
  2. Establish therapeutic relationships with youth, offering supportive services to enhance their functioning.
  3. Conduct housing assessments and ensure compliance with agency standards.
  4. Collaborate with youth and stakeholders to develop and implement Comprehensive Independent Living Plans.
  5. Maintain open communication with youth, families, and involved agencies.
  6. Consult with team members regarding youth progress in developing independent living skills.
  7. Fulfill case management responsibilities in accordance with policy.
  8. Meet regularly with youth to provide guidance and monitor progress.
  9. Facilitate referrals to social service agencies for additional support.
  10. Assist youth in exploring higher education options and completing necessary applications.
  11. Support youth in meal planning and grocery shopping as per policy.
  12. Help youth secure employment and provide job coaching.
  13. Ensure completion of the mandatory independent living curriculum.
  14. Identify and document intervention strategies.
  15. Provide transportation for youth to appointments as needed.
  16. Conduct regular life skills groups.
  17. Act as a liaison between various stakeholders, including schools and community agencies.
  18. Participate in meetings and hearings as required.
  19. Keep the Clinical Director informed of significant incidents and client progress.
  20. Complete all required reports in a timely manner.
  21. Attend professional development opportunities to maintain expertise.
  22. Schedule regular meetings with your supervisor.
  23. Perform additional duties as assigned by the Clinical Director.

On-Call Responsibilities:

  1. Be available to respond to crises as per agency policy.
  2. Assist staff with questions and ensure adherence to procedures.
  3. Remain reachable by phone after hours and within a reasonable distance from the campus.

Job Requirements:

  1. Must be at least 21 years old.
  2. Possess a valid Kentucky driver's license.
  3. Successfully pass a comprehensive background check.
  4. Hold a Bachelor's Degree in a related field.
  5. Complete required annual training based on education level.
  6. Be trained in CPR and First Aid, maintaining current certification.

Physical Requirements:

This role involves frequent communication via phone, computer use, filing, public speaking, and mobility throughout the facility. Case Managers must also be able to transport youth as necessary. Physical capability to perform required training is essential.
