Job Summary

Saint Vincent Hospital is seeking a skilled Patient Care Coordinator to join our team. As a key member of our healthcare team, you will play a vital role in ensuring that patients receive the highest quality care and support.

Key Responsibilities
  • Provide navigation assistance to patients receiving care in assigned areas
  • Identify patients with specific diagnoses and connect those patients to coordinated care management
  • Provide referrals, appointment scheduling, and/or lifestyle education to patients as specified in program procedures
  • Communicate with patients about care management options and model while offering referrals as needed
  • Maintain patient confidentiality and adhere to HIPAA laws
  • Provide excellent customer service and collaborate with care team members to ensure consistency and quality of care
Requirements
  • High school diploma or equivalent
  • 1 year related experience in an outpatient clinic or hospital setting, including 6 to 12 months data entry experience
  • Preferred: 1 to 2 years of work experience in insurance, medical records, billing, or related area
  • Medical Assistant certification preferred
