Job Description:As a Patient Care Coordinator at Kaiser Permanente Richmond Medical Center - KP, you will play a vital role in ensuring the smooth transition of patients from hospital to home. Your primary responsibility will be to coordinate and plan patient discharge processes, working closely with healthcare teams to ensure optimal patient care.
You will be responsible for managing critical need cases with efficiency and compassion, effectively communicating with patients and their families regarding care plans. Additionally, you will ensure compliance with healthcare regulations and standards, monitor patient progress, and adjust care plans as necessary.
Key responsibilities include:
