Job Title: Patient Care Coordinator

Stanford Health Care is seeking a highly skilled Patient Care Coordinator to join our team. As a Patient Care Coordinator, you will play a vital role in ensuring the smooth delivery of patient care services.

Key Responsibilities:
  • Prepare patients for exams and procedures by gathering necessary information and equipment.
  • Assist physicians with procedure set-ups, patient procedures, and examinations.
  • Perform patient care tasks, including vital signs, weights, rooming, and assisting with procedures.
  • Administer medication following approved procedures and only after checking medication and dose with a licensed person.
  • Process requests for prescription refills according to established practice.
  • Collect and send specimens to appropriate labs for testing per protocol.
  • Assist the physician with maintaining preventive health measures for a panel of patients.
  • Promptly and accurately report and refer patient concerns and symptoms to licensed clinic personnel.
Requirements:
  • High School Diploma or GED equivalent required.
  • Medical Assistant Certificate/Diploma from an approved school/institution or equivalent documented training required.
  • 1-5 years of directly related experience required.
  • BLS - Basic Life Support required.
  • CMA - Certified or Registered Medical Assistant preferred.
