About the Role

We are seeking a highly skilled and compassionate Registered Nurse Case Manager to join our team at Agape Hospice Care Atlanta. As a RN Case Manager, you will play a vital role in providing exceptional patient care and support to individuals and their families in need of hospice services.

Key Responsibilities
  • Patient Care and Support: Provide direct patient care services, including assessments, care planning, and implementation of individualized care plans.
  • Interdisciplinary Team Collaboration: Work closely with an interdisciplinary team of healthcare professionals to ensure comprehensive and coordinated care.
  • Case Management: Manage patient clinical records, ensure accurate and timely documentation, and maintain compliance with regulatory requirements.
  • Education and Support: Educate patients, families, and caregivers on disease management, symptom control, and end-of-life care.
  • Communication and Liaison: Serve as a primary liaison between patients, families, and healthcare providers to ensure seamless communication and coordination of care.
Requirements
  • Education: Registered Nurse of an accredited school of nursing with a current license to practice professional nursing in the state of Georgia.
  • Experience: One (1) year of experience in oncology, long-term care, hospice, or home health setting.
  • Certifications: Current Cardiopulmonary resuscitation (CPR) certification.
  • Transportation: Must have reliable transportation, proof of current automobile insurance, and a valid driver's license.
