VITAS Healthcare is seeking a skilled and compassionate Registered Nurse to join our team as a Nurse Care Coordinator. In this critical role, you will be responsible for providing high-quality, patient-centered care to individuals and families facing terminal illness and bereavement.
Key Responsibilities- Case Management and Supervision: Coordinate care, assess patient and family needs, and implement the interdisciplinary plan of care.
- Compliance and Quality Assurance: Ensure adherence to VITAS policies, standards, and regulatory requirements, and participate in quality improvement initiatives.
- Patient Education and Support: Provide education and training to patients, families, caregivers, and referral sources on end-of-life care, pain management, and other relevant topics.
- Interdisciplinary Collaboration: Work closely with the interdisciplinary team to develop, coordinate, and follow individualized plans of care.
- Communication and Documentation: Maintain effective communication with patients, families, and healthcare professionals, and ensure accurate and timely documentation of patient information.
- Professional Development: Participate in ongoing education and training to enhance skills and knowledge in end-of-life care and hospice services.
- Licensure and Certification: Current licensure to practice nursing in the state where the VITAS program is located.
- Experience: A minimum of two years of nursing experience in hospice, home health, or community health in the last five years.
- Skills and Abilities: Strong communication and interpersonal skills, ability to work independently and as part of a team, and proficiency in electronic medical records and other relevant software systems.
