Job Summary

The Registered Nurse Case Manager (RN Case Manager) will provide direct patient care services to hospice patients and supportive care to the patient's family; manage an interdisciplinary group (IDG) consisting of Licensed Practical/Vocational Nurses, Certified Nursing Assistants and support staff to enhance the quality of hospice care; and case manages all assigned patient clinical records.

Key Responsibilities
  • Promote and practice the company's philosophy and mission of providing compassionate, comprehensive, and qualified care to individuals with a life-limiting illness.
  • Assure the highest quality of care is provided and documented in accordance with the Plan of Care (POC).
  • Perform initial and on-going assessments of patients to identify their needs and develop effective care plans.
  • Develop and implement the POC in conjunction with the physician and other members of the IDG.
  • Provide education and support to patients, families, and caregivers on medication administration, dressing changes, oxygen administration, hospice philosophy, and care of the terminally ill patient.
  • Work closely with the IDG to provide all-inclusive physical, psychosocial, educational, emotional, and spiritual services, including bereavement support.
  • Maintain primary control and professional management of each patient and act as primary liaison between physician, patient, family, and hospice team.
  • Report any changes in patient condition to the Attending Physician and Hospice Physician.
  • Maintain close contact with patients' families and/or significant others to provide information, support, and continuity of care.
  • Maintain accurate and updated medical records, including IDG notes, POCs, initial and on-going assessments, and other pertinent information.
  • Perform on-going review and follow-up on assigned patient clinical records to ensure quality services are provided and standards of practice are followed.
  • Practice infection control measures and standard precautions.
  • Makes routine supervisory visits of and with all other team members at least monthly.
  • Maintains a high level of knowledge pertaining to the management of pain and symptoms in the dying process.
  • Participates in the company's Quality Assessment and Performance Improvement Program.
  • Attends staff meetings and may be requested to participate in the company's on-call schedule.
  • Consistently promotes the company's core values and completes annual training.
Requirements
  • Registered Nurse of an accredited school of nursing with a current license to practice professional nursing in the state in which we do business.
  • One year of experience in oncology, long term care, hospice, or home health setting.
  • Current Cardiopulmonary resuscitation (CPR) certification.
  • Must have reliable transportation, proof of current automobile insurance, and a valid driver's license.
  • Hospice experience preferred.
