This role involves performing advanced vocational nursing tasks, ensuring the care, treatment, and overall welfare of individuals residing at the SSLC with complex medical, mental health, and/or behavioral health needs.

Responsibilities include executing both routine and complex nursing tasks to enhance the health of individuals within designated areas and shifts.

Collect specimens for laboratory analysis and conduct health screenings.

Provide guidance to direct support professionals and other staff on delivering care that meets the medical needs of individuals.

Execute technical nursing procedures in compliance with facility and state regulations, while remaining informed of and adhering to all agency, SSLC, and unit/home policies and procedures.

Offer direction and support in on-the-job training for new LVNs and direct care staff.

Collaborate with Registered Nurses (RNs) to implement protocols and programs aimed at enhancing the health and safety of both staff and individuals.

Work under the general supervision of an RN, exercising moderate initiative and independent judgment.


Key Responsibilities:

Regular attendance is essential, with potential requirements for specific shift schedules, rotating shifts, extended hours, and/or overtime as per agency policy.

May participate in an on-call rotation as necessary to meet the operational needs of the facility.


Administer medications and treatments through various methods, including oral, rectal, internal, or injection.

Assist other medical personnel, such as RNs, Physicians, Advanced Practice Registered Nurses, and Physician Assistants, in patient examinations and care.

Conduct rounds and/or home visits as required, and may work in different locations within the SSLC as needed.

Demonstrate proficiency in performing complex technical nursing tasks in accordance with HHSC, SSLC, and state regulations.

Train and instruct new LVN staff on nursing functions at the SSLC, including familiarizing them with individuals and their specific needs and preferences.

Execute advanced nursing procedures as per facility and state guidelines, including tracheostomy care, gastrostomy/jejunostomy care, wound care, and breathing treatments.

Collect specimens for laboratory testing and perform necessary screening tests as requested by lab personnel.


Monitor individuals' weight, dietary intake, feeding routines, and vital signs, and assess health status post-treatment and procedures.

Instruct direct support professionals on monitoring individuals following treatments or procedures using provided instruction sheets.

Ensure timely and accurate documentation of treatments and observations in individuals' medical records, adhering to SSLC protocols.

Recognize symptoms of illness and report findings to RN and/or Primary Care Provider.


Foster interdisciplinary collaboration and effective communication with team members, ensuring that the treatment team has the most accurate information for decision-making.


Serve as a resource and mentor for new LVNs, assisting RN Supervisors with nursing instruction and training.

Support RNs in implementing protocols and programs to enhance health and safety.

Collaborate with the Infection Control Nurse to implement disease prevention measures and assist in quality assurance program development.

Collect data for RN/Primary Care Provider as needed.

Provide education to individuals in accordance with their care plans during procedures and treatments.

Ensure proper positioning of individuals prior to treatments and assist or instruct staff in repositioning as necessary.


Maintain an adequate supply inventory for required procedures and treatments, ordering supplies through appropriate channels when necessary.


Respond to emergency situations to ensure the safety of individuals, adhering to safety and infection control protocols.

Perform additional duties as assigned, which may include working in the clinic or lab, assisting with patient scheduling, or accompanying individuals off-site for activities or appointments requiring nursing attendance.


Provide treatment to HHS employees injured in the course of their employment.

Other duties may include participating in disaster response and recovery efforts or Continuity of Operations (COOP) activation, which may require alternate shift patterns or locations.


Required Knowledge, Skills, and Abilities:

  • Understanding of nursing care standards, procedures, and techniques, along with principles for delivering appropriate patient care.
  • Familiarity with emergency procedures and equipment, such as CPR, and the ability to assess and care for residents and staff in emergencies.
  • Strong reasoning skills for problem identification and resolution.
  • Proficiency in computer operation and relevant software.
  • Competence in patient care and treatment.
  • Ability to provide care to a diverse range of individuals with complex medical, mental health, and/or behavioral health issues.
  • Skills in record preparation and maintenance, including use of electronic health record systems.
  • Effective communication skills for instructing others.
  • Ability to interact appropriately with individuals with intellectual/developmental disabilities, their families, and staff.
  • Current CPR certification is required, along with the physical capability to perform CPR safely.
