POSITION SUMMARY:

Under the guidance of the Director of Nursing, the Licensed Vocational Nurse delivers nursing care to residents as directed by the physician and in alignment with nursing standards and regulations. This role also includes supervising the Certified Nursing Assistant (CNA) staff in providing direct care to residents.

ESSENTIAL POSITION FUNCTIONS:
  1. Review and prepare medical documentation and nursing notes, assessing both the physical and emotional well-being of residents. Monitor resident activities, including vital signs, and deliver nursing care as per physician's directives, care plans, established standards, and facility protocols. Facilitate and assist with new admissions, transfers, or discharges. Transcribe physician's verbal orders. Communicate effectively with residents, family members, interdisciplinary team members, and management regarding resident status.
  2. Administer medications as prescribed by the physician, adhering to nursing standards and facility policies. Ensure all charting and record-keeping is completed accurately and in accordance with established procedures.
  3. Understand, uphold, and advocate for all regulations concerning residents' rights.
  4. Assist in creating and implementing individualized care plans that cater to the specific needs of each resident.
  5. Engage in in-service training; support the implementation of orientation programs and job skills training; maintain professional development.
  6. Oversee the environment and care practices of nursing staff to ensure compliance with safety, fire, disaster, infection control, and other departmental policies; maintain cleanliness and safety in work and treatment areas.
  7. Supervise the CNA staff on the unit, including monitoring work assignments and providing constructive feedback and guidance as necessary.
  8. Perform other responsibilities as assigned.
SUMMARY OF QUALIFICATIONS:
  1. Must possess a valid Licensed Vocational Nurse (LVN) credential.
  2. Ability to establish positive relationships with residents and families, and collaborate effectively with other staff members.
  3. Must demonstrate regular attendance.
  4. Capable of supervising other staff and working collaboratively to achieve resident care goals.
  5. Physical and Sensory Requirements (with or without mechanical assistance): Ability to walk, reach, bend, lift, grasp, coordinate fine motor skills, push and pull; ability to hear and respond to alerts; ability to distinguish various smells, tastes, and temperatures; ability to read and write; ability to understand and follow both written and verbal instructions; ability to communicate effectively with residents, families, staff, vendors, and consultants; ability to maintain composure under pressure.
  6. Must comply with all local health regulations and successfully complete a post-offer health assessment.
  7. Must be capable of performing the essential functions of this position, with or without reasonable accommodation.
