Fertility Care Coordinator

Role OverviewThe Patient Navigator is a key role in our patient's journey, providing exceptional care and support throughout their treatment plan. As a Patient Navigator, you will be the primary point of contact for patients, guiding them through their treatment plan and providing education on procedures, testing, and follow-up care.
Responsibilities- Coordinate and schedule patient appointments, tests, and referrals to ensure a smooth continuum of care.
- Assist patients with understanding their medical care, insurance coverage, and available financial resources.
- Communicate with healthcare providers, clinical staff, and administrative teams to ensure seamless patient care coordination.
- Maintain accurate and up-to-date patient records, including documentation of interactions and care plans in the electronic medical record (EMR) system.
- Provide emotional support and resources to patients and their families, addressing any concerns or barriers to care.
- Participate in team meetings, case conferences, and quality improvement initiatives to enhance patient care delivery.
- Educate patients on treatment protocols, self-care strategies, and medication usage.
- Track patient progress, follow up on missed appointments, and ensure compliance with treatment plans.
- Advocate for patients' needs and help resolve any issues or barriers to care.
- Bachelor's degree in healthcare, social work, or a related field preferred or a Certified Medical Assistant Certification (CMA).
- Minimum of 2 years of experience in customer service, patient navigation, case management, or a related healthcare role.
- Strong knowledge of healthcare systems, insurance processes, and patient advocacy.
- Excellent communication, organizational, and problem-solving skills.
- Proficiency in using electronic medical records (EMR) and other healthcare software.
- Empathetic, patient-focused, and able to work collaboratively in a multidisciplinary team.
Pinnacle Fertility offers a competitive compensation package, including a salary range of $24.00-$26.00 per hour, and a comprehensive benefits package, including healthcare, dental, life, and vision insurance, generous paid time off (PTO), paid holidays, and a retirement savings program.
