Job Summary

The Hospice Chaplain provides spiritual care services to patients and families/caregivers of Capital Hospice & Palliative Care LLC. This role involves direct patient care, collaboration with the Interdisciplinary Team, and coordination with other health care providers in various care settings.

Key Responsibilities:

  • Provide spiritual care services to patients and families/caregivers
  • Collaborate with the Interdisciplinary Team to develop and implement patient care plans
  • Coordinate with other health care providers to ensure comprehensive care
  • Engage patients and families in goal-setting and decision-making processes
  • Respect patients' and families' goals, values, and beliefs

Requirements:

  • Graduate of an accredited seminary or school of theology (BD, MDiv or equivalent theological degree)
  • Experience working with death and dying, individuals/family/caregivers
  • Ability to work as a member of an interdisciplinary group
  • Comfort in an interfaith setting
  • Ability to accept different lifestyles, cultures, beliefs, and values
  • Ability to network with community clergy and congregations
  • Knowledge of and commitment to hospice philosophy of care
  • Hospice experience preferred
  • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
  • Must successfully pass agency-required background checks
  • Must comply with agency health screening and immunization requirements
  • Requires travel to multiple work locations
