LPN - Per Diem Job Description

We are seeking a compassionate and skilled LPN to join our team at Pomperaug Woods, a senior living community that values hospitality and resident engagement.

Key Responsibilities:
  • Provide direct nursing care to residents, including assistance with Activities of Daily Living (ADL) and promoting independent lifestyles.
  • Identify and respond to resident medical and behavioral changes, developing and monitoring care plans as needed.
  • Collaborate with other disciplines to ensure comprehensive care and coordinate services.
  • Communicate effectively with residents, families, and staff to ensure seamless care delivery.
Requirements:
  • Current nursing license valid in the state of CT.
  • Ability to work in a fast-paced environment and respond to emergencies as needed.
  • Strong communication and interpersonal skills, with a focus on providing exceptional resident care.
