Job Summary:

The MDS / RN Coordinator is responsible for overseeing the Resident Assessment Instrument (RAI) process, ensuring timely completion of Minimum Data Set (MDS) assessments, Care Area Assessments (CAAs), and care plans. This role requires strong communication skills to work with nursing staff, family members, and residents to implement care plans and ensure accurate documentation.

Key Responsibilities:

  • Oversee the RAI process, ensuring schedules are followed and assessments are completed in a timely manner.
  • Collaborate with nursing staff to develop and implement care plans that meet the needs of residents.
  • Verify accuracy of MDS assessments and make necessary corrections.
  • Provide education to staff on ADLs, MDS, and other relevant topics.
  • Monitor documentation of Medicare and managed care residents, ensuring accuracy and compliance with regulations.
  • Participate in Quality Improvement initiatives to improve care outcomes and five-star ratings.
  • Stay up-to-date with regulatory requirements and industry best practices through ongoing education and training.

Requirements:

  • 2-4 years of experience in MDS and long-term care.
  • Point Click Care experience preferred.
  • RN degree from an accredited college (minimum ASN).
  • Current registration in the state.
  • AANAC certification.
  • Current CPR certification.
