About the Role

We are seeking a skilled Medical Social Worker to join our Hospice Home Care team. As a Medical Social Worker, you will provide social work services to patients in their place of residence, working closely with the Clinical Manager and other healthcare professionals to develop and implement individualized care plans.

Responsibilities
  • Provide social work services to patients and their families, including assessment, counseling, and advocacy.
  • Develop and implement individualized care plans that address the patient's physical, emotional, and social needs.
  • Collaborate with the healthcare team to ensure seamless care coordination and communication.
  • Document all patient interactions and services provided in accordance with company policy.
  • Participate in patient care conferences and contribute to the development of the plan of care.
  • Supervise assistant personnel as required by law.
  • Perform other duties as assigned and within the scope of practice.
Qualifications
  • Master's or Doctoral degree from a school of social work accredited by the Council on Social Work Education.
  • One year of social work experience in a healthcare setting, with three to five years preferred.
  • One year in home health care preferred.
  • Valid license from the state practicing in.
  • Current CPR certification.
  • Basic clinical skills.
  • Professional appearance and demeanor.
  • General knowledge of computers and the ability to learn industry-specific software applications.
  • Excellent oral, written, and interpersonal communication skills.
  • Excellent organizational and time management skills with a keen attention to details.
  • Ability to work effectively in a team environment.
  • Ability to develop critical thinking skills, research situations, solve complex problems, and deal with a variety of issues.
