About the Role

We are seeking a skilled Medical Social Worker to join our Hospice Home Care team. As a key member of our care team, you will provide social work services to patients in their place of residence, working closely with our clinical staff to ensure comprehensive care.

Responsibilities
  • Provide social work services to patients in their place of residence, in accordance with physician orders and plan of care.
  • Participate in the coordination of company services with community resources and services, serving as liaison between patients or family/caregivers and community resources.
  • Maintain collaborative relationships with organization personnel to support patient care.
  • Observe confidentiality and safeguard patient information.
  • Develop and maintain a cooperative relationship with healthcare providers, communicating effectively and professionally.
  • Maintain current knowledge of Medicare Conditions of Participation for social work services.
  • Participate in in-services and/or continuing education programs to stay up-to-date on industry developments.
  • Participate in patient care conferences and maintain contact with community support groups to provide professional expertise.
  • Plan and coordinate all social services within the company, documenting all patient/family services provided as required by company policy.
  • Assist physicians and/or other care team members in understanding significant social and emotional factors related to health problems.
  • Participate in the development of the plan of care and prepare clinical and progress notes in accordance with company policy.
  • Participate with patient discharge planning and supervision of assistant personnel when required by law.
Qualifications
  • Master's or Doctoral degree from a school of social work accredited by the Council on Social Work Education.
  • One year of social work experience in a healthcare setting required, three to five years preferred.
  • One year in home health care preferred.
  • Valid license from the state practicing in.
  • Current CPR certification.
  • Basic clinical skills.
  • Professional appearance and demeanor.
  • General knowledge of computers and the ability to learn industry-specific software applications.
  • Excellent oral, written, and interpersonal communication skills.
  • Excellent organizational and time management skills with a keen attention to details.
  • Ability to work effectively in a team environment.
  • Ability to develop critical thinking skills, research situations, solve complex problems, and deal with a variety of issues.
