Social Worker Position Overview

We are seeking a skilled and compassionate Social Worker to join our team at Elderwood at Burlington. As a Social Worker, you will play a vital role in ensuring the health and well-being of our residents by providing social services and support.

Responsibilities:
  • Assist with the acclimation of new residents and develop and review plans of care to meet their long and short-range health care goals.
  • Complete SCREEN forms for new admissions and discharge processes as applicable.
  • Participate in decisions regarding discharge or transfer of residents to another level of care.
  • Document pertinent information regarding residents as required by department and facility policies and procedures.
  • Provide financial, personal, or group counseling for residents and their family members or sponsors.
  • Participate in in-service training programs as requested.
  • Assume a social work caseload of all residents on a nursing unit according to policies and procedures described in Section B of the Social Services Manual.
Qualifications:
  • Minimum of a bachelor's degree in social work or a bachelor's degree in a human rehabilitation counseling, and psychology from an accredited institution.
  • One year of supervised social work experience in a Long Term Care setting working directly with individuals.
  • Experience in casework is required.
  • Valid driver's license and clean driving record are required.
  • Work experience in the admission or discharge area of social work is highly desirable.
  • Desire and ability to work with the elderly and to cooperate with other staff members are necessary.
