  • Collaborates with healthcare teams to develop and implement discharge plans, ensuring timely and safe patient outcomes.
  • Reports observations and reactions regarding patients to the appropriate person, accurately and concisely documenting those observations.
Requirements
  • Masters of Social Work (MSW) degree
  • One year of related experience in the field of Social Work
  • Three years of preferred experience in the field of Social Work
