Social Worker MSW

This role focuses on safeguarding and promoting the welfare of patients, providing essential services as needed.
Key Responsibilities- Provides social work assistance to patients and families relating to illness, disease, hospitalization, and life events.
- Identifies, assesses, and coordinates cases, anticipating needs related to illness, hospitalization, finances, education, and home/community services.
- Collaborates with the healthcare team to develop and implement treatment plans, ensuring timely and safe discharges.
- Intervenes in situations where the treatment plan can be significantly impacted by providing patients/families with support and counseling regarding social, emotional, and/or financial issues.
- Reports cases of suspected abuse or neglect to the appropriate authorities.
- Masters of Social Work (MSW)
- One (1) year of related experience in the field of Social Work
- Three (3) years of related experience in the field of Social Work (preferred)
