Social Worker

As a Social Worker, you'll play a vital role in our care management team, working closely with patients, providers, and community organizations to address the complex health and social challenges faced by our Dual Eligible Special Needs Plan (DSNP) members.
Key Responsibilities:
- Participate in the care planning process, collaborating with Care Managers to assess, set goals, and develop interventions to address the social determinants of health needs in our members' individualized care plans.
- Conduct regular discussions and updates with providers, primary care physicians, Medical Directors, pharmacists, and care management staff regarding the status of our members and progress towards goals.
- Serve as a member advocate, ensuring our members receive all necessary care allowed under their benefit plan and as available through Medicaid benefits and/or other community resources.
- Develop relationships with hospital social workers and community resources, utilizing available data to assure appropriate care management of catastrophic, acute, and chronically ill members.
- Assist in the identification and reporting of potential quality improvement issues.
Requirements:
- Minimum of 2+ years' experience in medical social work or case management.
- Active and unrestricted LCSW or LMSW in NY.
- Ability to travel 50-75% within Buffalo area.
Education:
- Master's degree in Social Work.
Pay Range:
The typical pay range for this role is: $51,111.24. This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors, including experience, education, geography, and other relevant factors.
This position is eligible for a CVS Health bonus, commission, or short-term incentive program in addition to the base pay range listed above.
