We are seeking a skilled Social Worker to join our team at Living Legends Health. As a key member of our interdisciplinary team, you will work directly with residents to identify their psychosocial, mental, and emotional needs, providing access to services that meet those needs.

Job Summary:

The Social Worker will work in accordance with CMS and NYS regulations, adhering to facility policies. Your primary responsibility will be to foster a patient-centered care environment, enabling residents to maximize their individuality, independence, and dignity.

Essential Duties & Responsibilities:

  • Complete a social history and psychosocial assessment for each resident upon admission, identifying social, emotional, and psychological needs.
  • Determine the resident's code status and maintain advance directive paperwork/policies, ensuring 100% accuracy.
  • Participate in the development of a written, interdisciplinary plan of care for each resident, addressing psychosocial needs and goals.
  • Facilitate Care Plan meetings with residents, families, and the interdisciplinary team, ensuring quarterly, annual, and significant changes are addressed.
  • Provide therapeutic interventions to assist residents in coping with their transition and adjustment to long-term care, addressing social, emotional, and psychological needs.
  • Support and educate residents, family members, and significant others on placement and facility issues, referring them to appropriate services when necessary.
  • Act as staff liaison for resident counsel and family counsel groups.
  • Provide clinical interventions to address catastrophic events during the resident's stay.
  • Discharge planner, identifying residents' discharge plans and coordinating the discharge planning process, making referrals for home care services prior to return to the community.
  • Meet with residents with psychosocial needs, those self-directing care, and those needing additional support and TLC, providing counsel and support.
  • Complete progress notes, BIMs, Mood, Psychosocial, Behavior, and Discharge observations timely and accurately, in accordance with established policies and guidelines.
  • Complete sections of the MDS as assigned, timely and accurately.
  • Daily documentation on resident/representative interactions.
  • Facilitate room-changes.
  • Participate in QAPI, completing and presenting audits as assigned, and identifying issues related to resident wellbeing.
  • Complete all required documentation in the medical record (EMR).
  • Work collaboratively with members of all disciplines, internally and externally, to ensure the highest quality of care for residents and families.

Qualifications:

Skills: Expert assessment, communication, compassion, documentation, and people skills.

Education: Preferred Bachelor's or better in Social Work or Human Services.
