Job Summary

We are seeking a highly skilled and compassionate Social Services Professional to join our team at ECMC. As a key member of our interdisciplinary healthcare team, you will provide professional social work services to patients and their families, helping them navigate medically related social and emotional challenges.

Job Responsibilities

The ideal candidate will possess a Master's Degree in Social Work and be licensed as a Licensed Master Social Worker (LMSW) or a Licensed Clinical Social Worker (LCSW) issued by the New York State Department of Education. You will work closely with healthcare professionals to identify patient needs and develop restorative and discharge plans. Your duties will include:

  • Assessing patients' psycho-social needs upon admission and during their stay;
  • Establishing social work goals to aid patients, their families, or responsible parties in dealing with identified problems;
  • Providing counseling and supportive social work services to help patients adjust to their illnesses, disabilities, and stay in the facility or community;
  • Collaborating with facility staff to address the social and emotional aspects of patient healthcare and treatment;
  • Recording significant social services progress data and periodic reassessments of inpatient medical records;
  • Determining the psycho-social implications of potential discharge or transfer to different levels of healthcare and aiding facility staff and patient families in implementation;
  • Modifying social service goals or treatment consistent with changes in patient care needs;
  • Maintaining information on community and health resources for patient care and subsequent discharge;
  • Preparing various records and reports related to social services activities.
Requirements

To be successful in this role, you will need:

  • Good knowledge of social work practices and procedures, as well as social and psychological factors related to disease and disability;
  • Working knowledge of State and Federal rules, regulations, and requirements related to healthcare facilities;
  • Ability to counsel hospital patients and their families, function as a member of an interdisciplinary healthcare team, and interpret Federal, State, and local laws and regulations regarding social work services in healthcare facilities;
  • Ability to prepare records and reports related to social services activities, utilize community resources effectively, and confer with and provide social services consultation to other healthcare professionals;
  • Sympathetic attitude toward the sick and disabled, tact, and ability to perform the essential functions of the position with or without reasonable accommodation.
Compensation

The estimated salary for this position is in the range of $29.788 to $40.358 per hour, based on the job requirements and location. ECMC offers a competitive salary and benefits package, as well as opportunities for professional growth and development.
