Job Summary

VIA Visually Impaired Advancement is seeking a skilled Social Worker to join our team. As a key member of our Vision Rehabilitation Services department, you will play a vital role in providing exceptional support to individuals with visual impairments.

Your primary responsibilities will include conducting intakes, establishing Individualized Service Plans, and providing vision adjustment counseling and community linkages. You will work closely with our LCSWs/LMSWs to ensure seamless service delivery and achieve NYSCB contractual outcomes.

We are looking for a highly organized and empathetic individual with a strong knowledge of community resources and the social services process. If you possess excellent interpersonal communication skills, a caring and counseling approach, and the ability to work effectively with individuals of diverse backgrounds, we encourage you to apply.

Key Responsibilities:

  • Conduct intakes and establish Individualized Service Plans
  • Provide vision adjustment counseling and community linkages
  • Work closely with LCSWs/LMSWs to ensure seamless service delivery
  • Document progress and complete reporting within established guidelines
  • Maintain accurate electronic client files and data

Requirements:

  • Bachelor's in Social Work required
  • One-3 years' experience in the Social Work field preferred
