Responsibilities

The Clinical Care Coordinator is responsible for promoting optimal healthcare outcomes in alignment with UofL Health's policies, procedures, and values. Key responsibilities include:

  • Conducting thorough initial assessments for patient admissions to ensure appropriate care levels.
  • Collaborating with healthcare professionals to facilitate effective care coordination and discharge planning.
  • Utilizing the nursing process to assess, plan, implement, and evaluate patient care strategies.
  • Maintaining confidentiality and supporting organizational objectives by providing essential information to various departments.
  • Engaging in daily interdisciplinary rounds to optimize patient care and resource utilization.

Under the guidance of the Care Coordination Manager or Director, the Clinical Care Coordinator will:

  • Conduct timely assessments based on clinical indicators to validate patient demographics and insurance information.
  • Participate in the ongoing multidisciplinary assessment of patients to ensure high-quality care.
  • Develop and adjust individualized care plans based on patient needs and conditions.
  • Document all care activities professionally and accurately, adhering to quality review standards.
  • Advocate for patient and family needs in a respectful and confidential manner.
  • Serve as a liaison between patients and the healthcare team, addressing complex cases and educating staff on the role of case management.
  • Refer cases to appropriate management when necessary, ensuring compliance with care standards.
  • Deliver essential discharge information to patients, ensuring they understand their care plans.
Qualifications

Education:

  • Associate's Degree in Nursing required; must be actively pursuing a Bachelor of Science in Nursing or a related field.
  • Bachelor's Degree in Nursing or a related field preferred.

Experience:

  • Minimum of two years of experience as a Registered Nurse required.
  • One year of experience in discharge planning or case management in an acute care setting preferred.

Licensure:

  • Current Kentucky RN licensure or a compact license with privileges to practice in Kentucky required.

Certification:

  • Basic Life Support (BLS) certification from the American Heart Association required.
  • Case Management Certification (ACM, ANCC-Nurse Case Manager, or CCM) preferred.
