  • Realizar evaluaciones clínicas exhaustivas y específicas de enfermedades para todos los casos identificados, que incluyen, pero no se limitan a, la evaluación del estado físico, funcional, social y psicológico del miembro, así como sus necesidades culturales y lingüísticas.
  • Realizar evaluaciones post-alta para identificar las necesidades del miembro después de la hospitalización o el alta del departamento de emergencia.
  • Desarrollar e implementar un plan de atención específico para el miembro que incluya objetivos priorizados que sean Específicos, Medibles, Alcanzables, Relevantes y con un Tiempo definido (SMART).
  • Revisar, modificar y actualizar continuamente los planes de atención para reflejar las necesidades del miembro, al menos anualmente o cuando haya un cambio en la condición.
  • Coordinar la atención y los servicios con los miembros, sus familiares y otros proveedores, según sea apropiado.
  • Facilitar referencias a servicios de salud conductual y trastornos por uso de sustancias, así como a recursos comunitarios.
  • Colaborar con el personal interdepartamental en la resolución de casos según sea necesario.
  • Abogar en el mejor interés del miembro para obtener financiamiento necesario, alternativas de tratamiento y coordinación de atención.

Apoyo Administrativo (10%): Participar en una cultura de alto rendimiento centrada en el miembro, con un enfoque en el servicio al cliente, la consistencia, la dignidad y la responsabilidad.

Cualificaciones Mínimas:

  • Título de asociado en enfermería (ADN) o campo relacionado, más 3 años de experiencia clínica o experiencia en atención administrada.

Cualificaciones Preferidas:

  • Título de Licenciatura en Ciencias de la Enfermería (BSN) o campo relacionado.
  • Certificación en Gestión de Casos (CCM).
  • Bilingüe en inglés y uno de los idiomas definidos como umbral por el cliente.

Licencias / Certificaciones Requeridas:
