About the Role

Bishop Drumm Retirement Center is seeking a highly skilled MDS Coordinator to join our leadership team. As a key member of our clinical operations team, you will play a critical role in ensuring the accuracy and quality of our resident assessments and care plans.

Key Responsibilities
  • Resident Assessment and Care Planning: Conduct and coordinate the development and completion of resident assessments (MDS) in accordance with current rules, regulations, and guidelines.
  • Accuracy and Timeliness: Ensure accurate and timely completion of the MDS process to meet regulatory requirements.
  • Interdisciplinary Collaboration: Collaborate with the interdisciplinary team to develop individualized resident care plans that meet their unique needs and goals.
  • Progress Monitoring and Documentation: Monitor and document resident progress and responses to care plans to inform ongoing care and quality improvement initiatives.
  • Quality Assurance and Performance Improvement: Participate in the Quality Assurance and Performance Improvement (QAPI) process to identify areas for improvement and implement evidence-based solutions.
  • Communication and Coordination: Communicate effectively with residents, families, and other healthcare providers to coordinate optimal care delivery and ensure seamless transitions.
  • Regulatory Compliance: Stay up-to-date on MDS regulations and guidelines to ensure compliance and maintain the highest standards of care.

Requirements
  • Current RN License: A current Registered Nurse (RN) license in good standing.
  • RAC-CT Certification: RAC-CT (Resident Assessment Coordinator-Certified) certification is required.
  • Strong Clinical Knowledge: A strong understanding of MDS 3.0, RAI process, and care planning principles.
  • Excellent Communication and Interpersonal Skills: Excellent organizational, communication, and interpersonal skills to work effectively with residents, families, and healthcare teams.
  • Ability to Work Independently and Collaboratively: Ability to work independently and collaboratively within a team to achieve shared goals and objectives.
