We are seeking a highly skilled and compassionate Nurse Coordinator to join our team at the Department of Veterans Affairs. As a key member of our healthcare team, you will play a vital role in ensuring the seamless coordination of care for our nation's veterans.

Job Summary

The Nurse Coordinator will be responsible for coordinating care for veterans hospitalized in local community non-VA and DOD hospitals, documenting results, and following up until discharge. You will also coordinate patient transfers from both VA and non-VA hospitals to VA Puget Sound Inpatient medicine or surgery services. Additionally, you will complete consults/referrals for VISN Integrated Care (VIRS) and the Traveling Veteran program when assigned to those duties.

Key Responsibilities
  • Meet the needs of customers while supporting VA missions, including ICARE values, and consistently communicate and treat internal and external customers in a courteous, tactful, and respectful manner.
  • Coordinate care for veterans hospitalized in local community non-VA and DOD hospitals, document results, and follow up until discharge.
  • Coordinate patient transfers from both VA and non-VA hospitals to VA Puget Sound Inpatient medicine or surgery services.
  • Complete consults/referrals for VISN Integrated Care (VIRS) and the Traveling Veteran program when assigned to those duties.
  • Provide alerts and notifications for other VA staff, including providers and social workers, to ensure a holistic approach to care coordination.
  • Work daily with community care managers, social workers, providers, and nurses to assist in discharge planning and follow-up care, including VA services when applicable and appropriate.
Requirements
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
  • Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE).
  • English Language Proficiency. In accordance with 38 U.S.C. 7403(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
Preferred Qualifications
  • BSN Preferred
  • Care Coordination/Case Management experience
  • 5 years of Direct Patient Care Preferred
