Job Title: Nurse Practitioner

Monogram Health is seeking a skilled Nurse Practitioner to join our team of healthcare professionals. As a key member of our integrated Care Team, you will play a vital role in delivering high-quality, patient-centered care to individuals with chronic kidney disease and related metabolic disorders.

About the Role:

As a Nurse Practitioner at Monogram Health, you will be responsible for conducting comprehensive annual wellness exams, counseling and educating patients and families about benefits and programs available to help them live healthier lives, and documenting patient information in a timely and accurate manner.

Responsibilities:
  • Conduct assessments and comprehensive annual wellness exams on patients in their homes and virtually
  • Counsel and educate patients and families about benefits and programs available to help them live healthier lives
  • Document patient information in a timely and accurate manner
  • Coordinate care with primary care providers, specialists, and ancillary services
  • Deliver evidence-based, timely care that reduces avoidable hospitalizations and maximizes quality of life
  • Prescribe medications, order tests, and collaborate with patient's Monogram physician
  • Participate in integrated care team meetings and facilitate closing gaps in care
Requirements:
  • Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license
  • Board certified for appropriate licensure (NP: ANCC/AANP; PA: NCCPA)
  • Current and unrestricted DEA certificate
  • Ability to work without direct supervision and practice autonomously
  • Access to transportation, a valid driver's license, and car insurance
  • Knowledge of computer-based data management programs and information systems, as well as medical records and point-of-interview technology
  • Ability to communicate effectively in verbal and written form with retail and medical partners at various levels, patients, family members, physicians, and representatives of the community
  • Sound understanding of all federal and state regulations, including HIPAA and OSHA
  • 2 or more years of direct patient care required
  • Managed Care/IPA/Health Plan experience
  • Experience conducting annual wellness visits or similar comprehensive visits virtually or in the home
