Job Title: Medical Care Specialist

We are seeking a dedicated and compassionate Licensed Practical Nurse to join our team at Agawam East Rehab & Nursing.

Key Responsibilities:
  • Administer medications, provide wound care, and monitor vital signs
  • Conduct thorough assessments, develop individualized care plans, and implement interventions to promote optimal health outcomes
  • Maintain accurate records of patient information, including progress notes and medication administration records

Requirements:

Compensation: $60,000 - $80,000 per year, based on experience and benefits package, with possible shift differential add-ons.
