Shift: Mon-Fri 7am-4pm

This position presents a fulfilling opportunity to positively impact the lives of those in need. As a Case Manager, you will adopt a client-focused, strengths-oriented, and trauma-informed methodology to assist individuals and families in their pursuit of self-sufficiency.
ABOUT US: Established in 1979 in response to increasing hunger and homelessness in North San Diego County, Interfaith Community Services aims to disrupt the cycle of poverty and homelessness by empowering individuals to help themselves. As the leading nonprofit social services organization in North County, we provide both immediate assistance and long-term transformative programs that address the comprehensive needs of those in crisis. Collaborating with over 250 diverse faith communities, more than 650 dedicated volunteers, and a growing team of over 200 staff members, we support over 17,000 community members each year on their journey to self-sufficiency.
YOUR ROLE: This role involves delivering direct services to clients, which includes but is not limited to, conducting intake assessments, providing essential supportive services, advocating for clients (resources & referrals), and managing comprehensive vocational and housing stability cases, particularly assisting individuals transitioning from homelessness to stable housing while upholding the mission of Interfaith Community Services.
PRIMARY RESPONSIBILITIES:
Client Support:
- Monitor client progress, update case plans, and offer strengths-based case management.
- Ensure timely, comprehensive, and client-centered services.
- Conduct thorough assessments to identify individual needs and obstacles to stability.
- Provide strengths-based case management, including assessments, personalized service plans, crisis intervention, and advocacy.
- Guide and support clients in recognizing their strengths, establishing achievable goals, and working towards self-sufficiency.
- Create customized stability plans, including employment and housing strategies, offering ongoing support, tracking participant progress, and providing necessary resources and referrals.
- Document case notes, case plans, progress notes, and other required records accurately.
- Advocate for clients in various situations.
- Administer supportive and basic needs services according to agency and contract standards.
- Connect clients with vital resources within and outside the agency.
- Organize monthly meetings, training sessions, social events, and classes to engage clients.
- Collaborate with other agencies, faith centers, and community organizations to deliver services.
Housing Services:
- Link clients to supportive housing options and community resources, including healthcare, mental health services, and legal assistance.
- Provide housing navigation and support for housing instability, assisting participants with applications and eligibility for housing choice vouchers, emergency rental assistance, and other housing programs.
- Maintain regular communication with clients and the team regarding housing options and availability.
- Keep active housing listings and guide participants through application processes.
Employment Services:
- Employ a strengths-based approach to identify and enhance clients' employment skills.
- Facilitate the employment process, including connecting clients with job opportunities, registration, eligibility verification, job placement, referrals, workshops, and educational programs.
- Assist clients with resume development, interview preparation, and career planning.
- Connect individuals with job opportunities through our day labor program and job placement services, advocating for fair wages for participants.
Financial Empowerment:
- Help clients establish financial goals focused on budgeting, saving, and increasing income.
- Assist clients in applying for public benefits (CalFresh, General Relief, etc.).
Data & Reporting:
- Maintain accurate case information in electronic databases and client files.
- Complete necessary documentation and generate statistical reports promptly.
- Support the Program Manager in data collection for program reports.
QUALIFICATIONS:
- High School Diploma or equivalent required.
- Associate or Bachelor's Degree in Social Work, Health and Human Services, or a related field preferred.
- Minimum of 1 year of experience in a shelter, social services, or nonprofit environment required.
- Experience working with the homeless population preferred.
- Bilingual (Spanish/English) preferred.
- Valid California driver’s license and ability to operate a company vehicle required.
