Job Summary

We are seeking a highly skilled and compassionate Registered Nurse to join our team. As a Registered Nurse, you will be accountable for providing a restorative healing experience for patients and their support systems in partnership with the Clinical Manager and interdisciplinary team.

Key Responsibilities
  • Patient Assessment:
    • Perform thorough and ongoing holistic nursing assessments to identify current and potential problems.
    • Develop individualized plans of care that address patient needs and concerns.
    • Evaluate problems based on patient condition and policy.
  • Patient Care/Discharge Planning:
    • Collaborate with patients, support systems, and healthcare team members to meet patient needs and outcomes.
    • Implement plans of care upon admission or within 24 hours.
    • Facilitate evaluation, revision, and documentation of patient plans of care.
  • Teaching:
    • Provide individualized, ongoing teaching to patients and support systems throughout the continuum of care.
    • Utilize current, evidence-based practice when teaching patients and support systems.
  • Communication:
    • Demonstrate mutual respect and collaboration when communicating with the interdisciplinary healthcare team.
    • Utilize effective communication skills, such as active listening and assertive communication techniques.
  • Treatment/Procedures:
    • Perform treatments and procedures in accordance with established evidence-based policies and procedures.
    • Collaborate with the MD and healthcare team in the performance of these tasks.
  • Documentation:
    • Accurately and efficiently document pertinent patient information using the nursing process.
  • Medication Administration:
    • Provide all aspects of the medication process, including assessment, administration, monitoring, and evaluation.
    • Teach patients and support systems regarding safe medication use.
  • Quality Performance and Improvement (QPI):
    • Participate in the QPI process to achieve excellent patient outcomes and improved processes.
    • Develop QPI plans by identifying problems, collecting data, analyzing results, and implementing corrective action plans.
  • Delegation to and Development of Co-workers:
    • Accurately delegate patient care tasks to LPN, CNA, and other ancillary personnel.
    • Supervise the provision of care and provide direction and supervision as needed.

Qualifications/Requirements
  • Experience:
    • Basic knowledge of nursing theory and practice.
    • Knowledge of appropriate technology and practice standards for area of responsibility.
    • Demonstrates sound clinical skills and positive communication skills.
  • Education:
    • Graduate of an accredited school of nursing.
    • Currently licensed in the State of New York, or permittee.
    • Membership in a professional organization suggested.
  • Licenses/Certifications:
    • New York State Registered Nurse license required, or permittee.
  • Other:
    • Meets the criteria for Level II for CAP Program.
