At ChenMed, we take pride in our unique approach to primary care. The Nurse Practitioner (NP) plays a vital role within our clinical operations team, delivering direct patient care in our dedicated medical centers.


Key Responsibilities:

Conduct comprehensive geriatric assessments, gather medical histories, perform physical examinations, diagnose and treat conditions, and develop tailored nursing care plans. Provide health education, facilitate physician referrals, and manage case referrals while ensuring thorough documentation that aligns with ChenMed's standards for quality, service, productivity, and teamwork.

The Nurse Practitioner is expected to operate both independently and collaboratively with other healthcare professionals. Regular consultations with management, physicians, and medical directors are essential to maintain compliance with established guidelines.

This role may involve participation in risk management programs, clinical meetings, and other collaborative efforts. The Nurse Practitioner must adhere to departmental goals, performance standards, regulatory compliance, and policies to ensure quality patient care.

As a certified nurse practitioner, you will independently manage a patient population while collaborating with a licensed physician. Responsibilities include conducting and documenting physical assessments and patient histories, analyzing patient condition trends, and creating, documenting, and implementing management plans based on collected data.

Additionally, you will assist in developing nursing care plans and provide counseling and education to patients, families, and significant others.

Patient care planning will be based on a thorough understanding of the specific patient population and protocols, with attention to identifying physiological and psychological issues, considering the patient's cultural background, understanding level, personality, and support systems.

Referrals for specialized consultations may be necessary, including but not limited to dermatology, neurology, and psychiatry.

Independent performance of invasive procedures may be required upon successful completion of competency documentation.

Participation in patient care rounds and conferences is expected, along with effective communication of management strategies to the patient care team.

Collaboration with multidisciplinary team members is crucial to ensure successful patient management strategies that meet care needs.

Recognizing situations that require immediate physician attention and initiating life-saving procedures when necessary is a critical aspect of this role.

Working closely with other team members, you will analyze and evaluate current healthcare delivery systems to identify and implement improved practice patterns.

Collaboration with physicians, nurses, physical therapists, social workers, families, and caregivers is essential to facilitate timely transitions to lower levels of care when medically appropriate.

Enhancing collaborative relationships will empower patients and families to make informed decisions regarding care goals, palliative care, and hospice options.

Facilitating discharges to appropriate care levels and utilizing preferred providers for additional services is part of the role.

A proven track record of achieving clinical performance metrics is essential. Attention to detail is critical, particularly regarding patient charts, medications, and follow-up care.

Basic computer skills and familiarity with Microsoft Office Suite are required. This position may involve local travel.

A minimum of an ARNP or similar advanced nursing degree is required, along with at least one year of clinical experience preferred, though strong new graduates will be considered.
