The Advanced Practice Nurse (APN) plays a crucial role in delivering direct patient care within ChenMed's specialized medical centers. Key responsibilities encompass comprehensive geriatric evaluations, gathering medical histories, conducting physical examinations, diagnosing and treating conditions, formulating nursing care plans, providing health education, and coordinating referrals for case management. The APN is also involved in clinical rounds and meetings, ensuring thorough documentation in line with ChenMed's standards for quality, service, productivity, and teamwork.

CORE RESPONSIBILITIES:

  • Conducts independent assessments of both acute and chronic clinical issues. Documents physical evaluations and patient histories, analyzes patient condition trends, and develops and implements management plans based on collected data. Provides counseling and education to patients and their families.
  • Plans and executes patient care tailored to the specific needs of the patient population, anticipating potential physiological and psychological challenges while considering cultural backgrounds and support systems. Acts as a patient advocate.
  • Manages patient care through the following actions:
  • Issues orders for admissions, transfers, and discharges.
  • Orders and interprets necessary laboratory and diagnostic tests.
  • Prescribes appropriate medications and treatments.
  • Refers patients to specialists as needed, including dermatology, neurology, and more.
  • Maintains detailed documentation through progress notes and summaries.
  • May perform certain invasive procedures upon demonstrating competency.
  • Engages in patient care rounds and discussions, communicating management strategies to the care team.
  • Identifies situations requiring immediate physician intervention and initiates emergency procedures when necessary.
  • Utilizes advanced communication skills to resolve complex issues and enhance patient service.

ADDITIONAL RESPONSIBILITIES:

  • Collaborates with multidisciplinary team members to evaluate and improve healthcare delivery systems.
  • Participates in professional development activities to enhance personal and professional growth.
  • Works in partnership with physicians, nurses, and other healthcare professionals to ensure smooth transitions to lower levels of care when appropriate.
  • Introduces self to patients and families, clarifying the role of the Advanced Practice Nurse.
  • Facilitates family conferences to discuss treatment objectives, optimize resource use, and provide education.
  • Fosters collaborative relationships to empower patients and families in making informed decisions regarding care goals.
  • Ensures appropriate discharge planning and utilizes preferred providers for additional services.
  • Prescribes medications in accordance with state regulations.
  • Performs other duties as assigned by management.

REQUIRED SKILLS AND QUALIFICATIONS:

  • Proven track record of achieving clinical performance metrics.
  • Strong critical thinking and problem-solving abilities.
  • Excellent communication and interpersonal skills.
  • Effective time management skills and ability to perform under pressure.
  • Compassionate and empathetic demeanor with strong listening skills.
  • Detail-oriented with a focus on patient care documentation.
  • Basic computer proficiency and familiarity with Microsoft Office Suite.
  • This role may require local travel.

EDUCATIONAL AND EXPERIENCE REQUIREMENTS:

  • ARNP or equivalent advanced nursing degree is mandatory.
  • Current Nurse Practitioner Certification in the relevant state is required.
  • Board certification by AANP or ANCC is preferred, depending on state regulations.
  • A current DEA number for controlled substances may be necessary based on state laws.
  • A minimum of one year of clinical experience is preferred, though strong new graduates will be considered.
  • Basic Life Support (BLS) certification from a recognized organization is required within the first 90 days of employment.
