Key Responsibilities
  • Engage Clients in Care Coordination: Develop and implement individualized care plans that address the unique needs and goals of each client.
  • Coordinate Healthcare Services: Work with clients, healthcare providers, and community resources to ensure seamless access to physical and behavioral healthcare services.
  • Provide Education and Support: Educate clients and their families on available resources, services, and support options to promote self-advocacy and empowerment.
  • Collaborate with Interdisciplinary Teams: Work closely with multidisciplinary teams, including healthcare providers, social workers, and other professionals, to ensure comprehensive care and support.
  • Document and Report: Maintain accurate and timely records of client interactions, progress, and outcomes, and report to supervisors and stakeholders as required.
Requirements
  • Education: Bachelor's degree from an accredited college or university in a social, behavioral, or human services field.
  • Experience: At least one year of experience in a related field, preferably in care coordination, social work, or a related field.
  • Skills: Excellent communication, organizational, and time management skills; ability to work autonomously and as part of a team; and proficiency in Microsoft Office and other relevant software.
