Job Summary

We are seeking a skilled Healthcare Coordinator to join our team at UnitedHealthcare. As a key member of our Integrated Care Model, you will play a vital role in ensuring our members receive the best possible care.

Key Responsibilities
  • Assess and develop individualized care plans for members, focusing on the most appropriate and least restrictive level of care.
  • Identify and initiate referrals for social services, including financial, psychosocial, community, and state supportive services.
  • Manage care plans throughout the continuum of care, serving as a single point of contact for members and stakeholders.
  • Communicate effectively with all stakeholders to ensure timely and quality coordinated care and services.
  • Advocate for patients and families to ensure their needs and choices are represented and supported by the healthcare team.
Requirements
  • Associate's degree or higher.
  • 4+ years of experience in a community health setting, with a focus on healthcare roles.
  • Intermediate Microsoft Office skills, including MS Word, Excel, and Outlook.
  • Experience in long-term care, home health, hospice, public health, or assisted living.
  • Ability to travel in assigned regions to visit Medicaid members in their homes and/or other settings.
Preferred Qualifications
  • Licensed social worker or behavioral health/clinical degree.
  • Experience working with the brain injury population.
  • 2+ years of case management experience.
  • 1+ years of experience working with Medicaid Waiver services.
  • Experience arranging community resources.
  • Background in managing populations with complex medical or behavioral needs.
  • Field-based work experience.
  • Experience with electronic charting.
