Overview:
As a Primary Care Specialist with HarmonyCares, you will provide high-quality, coordinated healthcare to patients in the comfort of their own homes. Our team is dedicated to delivering exceptional patient care, and we are seeking a skilled Nurse Practitioner to join our ranks.

To bring personalized, quality-based healthcare to the home of patients who have difficulty accessing care.

Responsibilities:
As a Primary Care Specialist, you will be responsible for providing primary care to high-acuity patients in their homes, Monday through Friday. You will work closely with our support team to ensure that each patient receives the highest level of care. We are looking for a clinician who is passionate about patient care and is committed to delivering exceptional results.

Qualifications:
To be successful in this role, you will need to be licensed as a Nurse Practitioner in the state where our office is located, have a federal DEA license, and have geriatric training or experience. You will also need to be certified in CPR/BLS. If you are a motivated and compassionate healthcare professional who is looking for a challenging and rewarding role, we encourage you to apply.
