Physical Therapist Home Care Specialist

AdventHealth is seeking a skilled Physical Therapist to join our Home Care team in Central Florida. As a Physical Therapist Care Manager, you will coordinate and direct the care of a caseload of home patients requiring physical therapy as the primary service.
Key Responsibilities:
- Coordinates and directs the care of a caseload of home patients requiring physical therapy as the primary service.
- Provides comprehensive assessment, planning, implementation, and evaluation for the caseload as the primary Care Manager.
- Optimizes schedule daily to support productivity, efficiency, and maintain best practice visit utilization.
- Performs evaluations, assessing function using a method that objectively measures activities of daily living.
- Formulates a patient-specific plan of care in consultation with the patient, family, and physician.
- Reassesses the patient every 30 days, providing ordered therapy service, functionally reassessing the patient, and comparing the resultant measurement to prior assessment measurements.
Requirements:
- Valid Driver's License
- Physical Therapist License
- CPR Certification
- Minimum of one-year relevant clinical physical therapy experience
- Graduated from an approved physical therapy curriculum as required by the state which issued the license
Preferred Qualifications:
- Recent, relevant experience in a Medicare-certified home health agency as a care-manager
- Master's or Doctorate in Physical Therapy
- Home Health Care-Manager Certification or COS-C
- Working knowledge of community resources and an ability to refer patients and families appropriately
- Home Care Regulations and Third-Party Reimbursement as it impacts care delivery
