Responsibilities

  • Act as a clinical resource for patients requiring additional evaluation and treatment via telehealth
  • Collaborate with local clinical teams to provide comprehensive care and coordinate follow-up visits
  • Address time-sensitive medication refills, lab results, and other patient needs
  • Interact with primary care providers to escalate or evaluate patients in emergency situations
  • Assist in service recovery for patients requiring timely care
  • Work closely with the pharmacy team to ensure comprehensive patient care
  • Frequently review quality metrics and identify areas for improvement to achieve superior performance
  • Educate patients on the Urgent Care Triage program to reduce hospital readmissions and improve outcomes
  • Identify gaps in care and assist in ordering/coordinating external resources as needed
  • Collaborate with the Regional CMO and Medical Director to identify opportunities for care improvement

Qualifications

  • Must be a certified Nurse Practitioner with active licenses in coverage states
  • Must have an active DEA license
  • Excellent written and verbal communication skills
  • Ability to react decisively and quickly in urgent and emergent situations
  • Demonstrated experience with multi-tasking
  • Strong communication and organizational skills

Preferred Qualifications

  • 1 year of experience at HarmonyCares Medical Group
  • Experience with APRIMA EMR
  • Experience with telehealth and coordinating care virtually
