Key Responsibilities

The Licensed Practical Nurse (LPN) in our hospice program will be responsible for delivering high-quality patient care and executing skilled nursing tasks in patients' residences.
  • Provide skilled nursing services under the guidance of a registered nurse (RN) on a daily basis.
  • Administer prescribed medications and treatments in accordance with state regulations as needed.
  • Assist the physician and/or RN in performing specialized medical procedures as required.
  • Support patients with daily living activities and promote self-care practices.
  • Communicate with the patient's physician and relevant personnel regarding significant changes in the patient's health status.
  • Perform advanced wound care as required, provided in-service training is documented for specific procedures.
  • Follow physician orders within the scope of practice for an LPN, adhering to hospice care standards without altering the care plan.

Qualifications

Education: Associate Degree or equivalent certification

Licensure and Certification: Required

Licensure Requirements

  • Unrestricted licensure in the state of practice.
  • Current CPR certification is mandatory.
  • Possession of a valid driver's license and vehicle insurance, along with access to reliable transportation or public transit.
