About the Role

We are seeking a highly skilled Care Transitions Coordinator to join our team at Enhabit Home Health & Hospice. As a Care Transitions Coordinator, you will play a critical role in ensuring a seamless transition of care for our patients, promoting quality and efficiency in the delivery of post-acute care.

Key Responsibilities
  • Assist patients in navigating post-acute care, focusing on creating a positive impact on patient outcomes and referral source satisfaction.
  • Develop patient-centered transition plans, integrating evidence-based clinical guidelines, preventative guidelines, protocols, and other metrics to promote quality and efficiency in post-acute care delivery.
  • Represent Enhabit Home Health & Hospice in strategic relationships with health systems, hospitals, inpatient facilities, physicians, and physician groups, as well as executive-level opportunities.
Requirements
  • Must be a graduate of an approved school of nursing, therapy, or social work.
  • Must be licensed in the state where they currently practice.
  • Must have two years' demonstrated field experience.
  • Must have demonstrated experience and understanding of federal, state, and local laws and regulatory guidelines governing the operations of Medicare-certified home health and hospice.
  • Must have basic demonstrated technology skills, including operation of a mobile device.
Preferred Qualifications
  • LPN Licensed Practical Nurse preferred.
  • Three years of field experience is preferred.
  • Previous experience in home health or healthcare sales is preferred.
Additional Requirements
  • Must possess a valid state driver's license.
  • Must maintain automobile liability insurance as required by law.
  • Must maintain dependable transportation in good working condition.
  • Must be able to safely drive an automobile in all types of weather conditions.
